The Report, Section by Section
- Referral question and background. Who referred your child, what concerns prompted the evaluation, and how the evaluation was conducted.
- Developmental and medical history. Birth, milestones, health, education, and family history, drawn from your intake interview and any records you provide.
- Assessments administered. The parent-rated measures and clinician-led activities used for your child, selected for their age and communication style.
- Behavioral observations. What the psychologist observed directly during the evaluation session and in the videos you uploaded.
- Results and clinical interpretation. Scores explained in plain language, with what they mean for your child.
- Diagnosis with diagnostic codes. The clinical conclusion, stated with the ICD-10 and DSM-5 language that payers and providers require.
- Recommendations. Broken out for school (IEP/504), therapy authorization, and home, with referrals written so providers can act on them.
